Hospice Registered Nurse (RN) Case Manager

We are looking for a compassionate, detail oriented registered nurse to join our team as a Hospice Registered Nurse (RN). The Hospice Registered Nurse (RN) oversees and coordinates the plan of care for a dedicated case load of patients. This means getting to know your patients, caregivers, and their families while working with a supportive and collaborative interdisciplinary team, focusing on holistic patient-centered care, and quality at end of life.
