Registered Nurse

Position Summary We are seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality home health services. The RN will be responsible for patient assessments, care planning, OASIS documentation, care coordination, and ensuring patients receive safe, effective care in their homes. Key Responsibilities Conduct initial and comprehensive nursing assessments, including OASIS documentation and medication reconciliation. Develop, implement, and revise individualized care plans in collaboration with physicians and the healthcare team. Provide skilled nursing care in accordance with physician orders and agency policies. Supervise Home Health Aides and LPN/LVNs, including required supervisory visits. Coordinate care with clinicians, physicians, patients, and family members. Maintain accurate and timely documentation in compliance with agency and payer requirements. Participate in on-call, weekend rotations, and potentially observed holidays as needed. Support quality improvement initiatives focused on patient safety and reducing hospitalizations. Assist with staff orientation, training, and mentorship when applicable. Required Qualifications Active, unencumbered Washington State RN License or Multi-State RN License. Current BLS Certification. Strong clinical assessment and care coordination skills. Ability to work independently and manage a patient caseload effectively. Preferred Qualifications Home Health nursing experience. Experience with HCHB/PointCare documentation systems. Excellent communication, organization, and time-management skills. #J-18808-Ljbffr
