LTSS Service Care Manager (RN)Location: Delaware (New Castle County) – Field-Based / Remote HybridDuration: 6 Months (Potential Extension/Conversion)Schedule: Monday – Friday, 8:00 AM – 5:00 PM ESTPay Range: $35 - $41 hourly on W2We are seeking an experienced LTSS Service Care Manager (RN) to support long-term services and supports (LTSS) members through comprehensive care coordination and case management. This role focuses on evaluating member needs, facilitating care plans, and ensuring access to appropriate healthcare and community resources to drive quality and cost-effective outcomes.
