Primary Care Provider

Medical Provider

The Primary Care Provider will provide exceptional health care to patients consistent with their training, experience-level, and Board licensure as well as in accordance with evidence-based guidelines and People's Center policies and procedures.

Qualifications:

• Doctor of Medicine, Doctor of Osteopathic Degree, Master of Science/Doctorate Degree in Nursing, Physician Assistant or Physician
• Current licensure to practice in the State of Minnesota
• Drug Enforcement Agency (DEA) license
• BLS Certification

• Competence with computer systems is essential to include familiarity with Microsoft Word, Power Point, Excel, and EPIC.
• Ability to inspire and motivate others and communicate the mission, vision, and the future of People's Center.

Responsibilities/Duties:

• Complete the credentialing and privileging process.
• Render appropriate, timely care to patients during their visit.
• Actively manage care when outside of the clinic with the assistance of the clinic team.
• Provide high quality care as demonstrated by chart reviews, quality measures, and clinical outcomes.
• Document each patient interaction in chart clearly and with appropriate detail.
• Document in a timely manner according to People's Center policies and procedures.
• Submit coding for the visit and any procedures performed, in accordance with clinical guidelines.
• Complete correspondence and forms in a timely fashion.
• Review test results in a timely fashion and communicate results to the patient.
• Review old records, consultations, and other outside records as they become available to the chart; maintain up to date patient problem list and medication lists.
• Return telephone calls to patients in a timely fashion, preferable on the day received, and document the conversation in their chart.
• Participate in quality assurance/quality improvement activities, as requested by People's Center.
• Demonstrate good stewardship of resources.
• Provide appropriate education to patients and families.
• Address, when feasible, social determinants of health with the assistance of team members.
• Follow universal precautions.
• Maintain best practice knowledge level by participating in continuing education opportunities and utilizing current professional reference materials; maintain continuing education requirements for licensure and Board certification; maintain affiliation with and evaluation of teaching programs as needed.
• Adhere to the standards and policies of People's Center, the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG, guidelines, and other State and Federal laws). This also includes reporting to the Compliance Officer, Privacy Officer, supervisor, and/or suggestion box, any suspected unethical, fraudulent, or unlawful acts or practices.
• Conduct timely quarterly peer chart reviews.

Physical Demands/Work Environment:

While performing the duties of this job, the provider is continually required to sit, actively listen, and communicate effectively with patients and other staff. The provider is occasionally required to stand, walk, use hands to finger, handle, and/or feel objects, tools, or controls, and reach with hands and arms.

The provider must occasionally lift and move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

The noise level in the work environment is moderately quiet.
