Remote | Part-Time W-2 Employee Role Overview

Avail Health is building a virtual behavioral health and dementia care program supporting Medicare beneficiaries with serious mental illness, cognitive impairment, dementia-related behavioral concerns, and complex psychosocial needs.

The Psychiatric Mental Health Nurse Practitioner (PMHNP) provides psychiatric evaluation, diagnosis, medication management, and longitudinal behavioral health support through a highly coordinated virtual care model. Working closely with therapists, care coordinators, dementia care specialists, and interdisciplinary clinical leadership, the PMHNP serves as a key clinical leader within a growing program designed to improve outcomes for vulnerable older adults.

This is a foundational role within a newly launching service line. The position offers flexibility, meaningful clinical impact, and the opportunity to help shape scalable behavioral health care delivery for Medicare populations.What You'll Own
  • Psychiatric evaluation and medication management for Medicare patients across California
  • Diagnosis and treatment of dementia-related behavioral symptoms, geriatric psychiatric conditions, and serious mental illness
  • Longitudinal behavioral health management in partnership with therapists, care coordinators, and dementia care specialists
  • Clinical decision-making and interdisciplinary collaboration supporting complex patient populations
  • Delivery of high-quality virtual psychiatric care within a technology-enabled care model
What You'll Do
  • Conduct comprehensive psychiatric evaluations via telehealth
  • Diagnose and treat dementia-related behavioral conditions, geriatric psychiatric disorders, and serious chronic mental illness
  • Develop and manage individualized treatment plans, including medication management
  • Provide ongoing psychiatric follow-up care and symptom monitoring
  • Collaborate closely with therapists, care coordinators, dementia care staff, physicians, and other interdisciplinary team members
  • Participate in interdisciplinary case review and care planning discussions
  • Escalate urgent psychiatric, behavioral, cognitive, or safety concerns appropriately to the care team
  • Utilize Avail's technology platform, including ambient AI documentation and clinical workflow tools
  • Schedule and manage follow-up appointments for assigned patients
  • Participate in required clinical meetings and team collaboration activities
  • Contribute clinical insight to support workflow refinement, documentation standards, and scalable behavioral health infrastructure as the program grows
  • Maintain timely, accurate, and compliant documentation in accordance with organizational, payer, and regulatory requirements

What You Bring
Required
  • Active, unrestricted California Nurse Practitioner license in good standing
  • Board Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
  • Minimum three (3) years of post-certification PMHNP clinical experience
  • Experience providing virtual care and telehealth services
  • Experience evaluating and treating older adults with psychiatric disorders, serious mental illness, and dementia-related behavioral conditions
  • Strong commitment to interdisciplinary collaboration and team-based care
  • Comfort utilizing technology-enabled clinical workflows and electronic health records
  • Reliable high-speed internet connection and dedicated HIPAA-compliant home workspace
  • Willingness to obtain additional state licensure as needed
Preferred
  • Experience working in senior living, assisted living, memory care, or long-term care settings
  • Experience assessing and treating patients with dementia and cognitive impairment
  • Familiarity with Medicare Advantage populations
  • Familiarity with RAF, HCC, value-based care models, and HEDIS measures
  • Active licensure in Maryland, Ohio, Indiana, or West Virginia
  • Experience working in startup or rapidly scaling healthcare organizations
Schedule and Work Style

Work Type: Fully remote telehealth delivery

Schedule: Approximately 10-20 hours per week based on patient volume and clinician availability

Hours: Flexible scheduling with Pacific Time-friendly patient hours

Travel: None required

Autonomy: High. Clinicians manage their own follow-up scheduling while working within a collaborative interdisciplinary care model.

Reports To: Ricardo Young, MDClinical Practice Model
  • Maximum of two patient appointments per hour
  • Initial psychiatric evaluations scheduled for 60 minutes
  • Clinicians manage their own follow-up scheduling
  • Integrated care model with therapy, care coordination, and dementia care support services
  • Ambient AI documentation and technology-enabled clinical workflows designed to reduce administrative burden
Compensation and Employment Details

Employment Type : Part-Time W-2 Employee

Compensation
  • $100 per hour for patient care services
  • $50 per hour for required training, meetings, and administrative activities
Additional Support
  • Collaborating physician provided by Avail Health
  • Professional liability and malpractice coverage provided ($1M/$3M)
  • Credentialing, compliance, billing, operational, and technology support provided

Benefits Eligibility
This position is not benefits eligible. Employees regularly scheduled to work 30 or more hours per week may become eligible for company-sponsored benefits in accordance with Avail Health benefit plan requirements and eligibility guidelines.About Avail Health
