Sierra has been part of the Las Cruces community since 1982, and our social workers are among the most trusted members of our interdisciplinary team.Key ResponsibilitiesConduct mental health and behavioral health assessments under a physician's plan of careProvide counseling, psychotherapy, and psychosocial support to patients and familiesSupport grief counseling, end-of-life decision-making, and advanced care planningContribute to diagnosis and care planning for patients with complex emotional and social needsCollaborate with the interdisciplinary team to coordinate care across clinical, emotional, and social domainsDocument all interventions, assessments, and care coordination per regulatory and agency standardsParticipate in QAPI activities, in-service training, and patient care planning meetingsOther Duties:Provide care and services in accordance with infection control, safety, and risk prevention protocolsAccurately document clinical interventions, care coordination, and communication in a timely mannerParticipate in interdisciplinary meetings, case reviews, and continuing education opportunitiesMaintain compliance with HIPAA and all company privacy and confidentiality standardsRepresent the organization positively and professionally in patients' homes and community settingsWork independently with minimal daily oversight and demonstrate initiative in completing responsibilitiesProvide courteous, timely, and professional customer service to patients, families, and partnersExhibit adaptability to changing responsibilities and flexibility in completing assignmentsDemonstrate consistent and effective use of organizational technology systems and tools required for the role, including communication, documentation, productivity, and data management platformsAdhere to all organizational policies, including job descriptions, mission, and the Employee HandbookCommunicate and interact professionally and respectfully with others to support team goalsPerform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the companyRequirementsMaster's degree in Social Work from a CSWE-accredited programCurrent, unencumbered LCSW license in New MexicoTwo or more years of clinical social work experience preferredDemonstrated competency in behavioral health, family dynamics, and counseling techniquesStrong documentation and communication skillsEMR proficiency and ability to learn hospice and home health documentation systemsReliable transportation, valid driver's license, and auto insurance for home visitsWhy Join UsSierra Health Care is a nonprofit — our mission is our bottom line.
