Overview
Physical Therapist – General, travel assignment in Ukiah, CA. Start date 08/31/2026 for 13 weeks. Weekly pay range $2,060–$2,118 with guaranteed 40 hours per week. Day shift, 5x8s, 8:00 AM–4:30 PM, with no weekend shifts. Setting: Short Term Acute Care Hospital. This assignment is managed by Healthforce Healthcare LLC.

Responsibilities
- Plan and conduct PT evaluations; develop individualized treatment plans to restore and improve physical function
- Deliver therapy interventions using evidence-based techniques and established treatment protocols
- Co-lead interdisciplinary care and supervise assigned assistants as applicable
- Establish and communicate discharge plans; coordinate with care teams and families
- Complete comprehensive clinical documentation within established timelines (EPIC experience preferred)
- Monitor progress, adjust therapies as needed, and ensure accurate patient education and continuity of care

Requirements
- Doctor of Physical Therapy (DPT) degree; required, only if the license was obtained on or after January 2016, or foreign equivalent
- Current Physical Therapy license in the state of practice
- Basic Life Support (BLS) or CPR certification from an approved vendor
- At least two years of physical therapy experience preferred; pelvic floor therapy experience required; PRPC or WCS certification or working toward certification
- EPIC EMR experience is required
- Ability to work in a Short Term Acute Care Hospital setting
