Unit Type: Case Management

Number of Beds: Units are mainly 20 beds each

Staffing: Team of Case Managers/Social Workers to manage the patient load

Shift Options:

Day Shift Friday to Monday, 8:00 AM to 4:30 PM

Day Shift Monday to Friday, 8:00 AM to 4:30 PM

Patient Ratios:

Social Worker: 1:40

Registered Nurse: 1:20

Type of Equipment: Mobile devices and computers

Electronic Medical Record (EMR): Meditech / CarePort

Required Skills:

Minimum of 1 year of current Case Management RN experience

Preferred Skills:

Discharge Planning experience

Emergency Department experience

Palliative Care knowledge and Goals of Care conversation

Responsibilities:

Conduct Discharge Planning Assessments

Manage Discharge Planning processes
