Summary:The Patient Navigator will create a welcoming environment for patients and guests of Alivio Medical Center (AMC) and Cook County Health & Hospital Systems (CCHHS). The Patient Navigator shall be highly visible customer serve role that provides a friendly experience when escorting patients, family members, and guest to their designated destination upon arrival and after registration.To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.ESSENTIAL DUTIES AND RESPONSIBILITIES:Cordially greets patients and guests, ascertains their needs, and directs them accordinglyProvides timely escort services to incoming patients and visitors to designated destinations throughout CCHHS and AMCCommunicates with patients and guests In a sincere friendly mannerFunctions as a resource for patients and guests seeking information, assistance, and recommendations.Assist patients and guests by responding to inquires and providing accurate information about CCHHS and AMC servicesAssists patients and guests with checking in at the designated kiosksProvide follow-up in-home visitsIdentifies and escalated priority issues, as neededPerforms other related duties assigned.Benefits:Full-Time Benefits:Paid Time off (Vacation, Paid Sick Leave and Paid Leave)Full Health Benefits (Medical , Dental , Vision, Disability, Life Insurance. )403B Retirement Plan.SUPERVISORY RESPONSIBILITIES:Manages and supervises staff listed below.NoneKEY INTERRELATIONSHIPS:WORKING CONDITIONS:OSHA Category 2 – Involves no regular exposure to blood, body fluids, or tissues, but may require unplanned tasks that involve exposure to blood, body fluids, or tissues.QUALIFICATIONS:Education:H.S. Excellent customer service skills. Ability to establish and maintain effective working relationships with patients, employees, and the public. Ability to communicate with people from various educational levels and backgrounds as well as the ability to speak clearly and concisely. Ability to stand and walk for prolonged periods of time.
