Responsibilities * Performs patient start of care comprehensive assessment, evaluation, development and implementation of plan of care. * Completes OASIS assessments timely and accurately. * Provides skilled nursing care to patients in accordance with the plan of care and agency policies. * Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources. * Initiates appropriate referrals when indicated. * Promotes and maintains patient health through teaching and appropriate rehabilitative measures. * Assists patients in learning appropriate self-care techniques. * Other duties as assigned. Qualifications * Current RN license, specific to the state(s) you are assigned to work. * One year of clinical experience as an RN. If less than one year clinical experience, you must be approved by VP of clinical. * Current CPR certification. * Valid driver's license, reliable transportation and liability insurance.
