ChenMed is opening a new physician position in Memphis, TN area, for a Primary Care Physician to serve seniors in a value-based care model. The role involves managing a patient panel of up to 450 seniors, leading a care team, and focusing on proactive care and health coaching for Medicare Advantage patients. Qualifications include an MD or DO in Internal Medicine, Family Medicine, or Geriatrics, with board certification preferred, and at least two years of clinical experience. About the role Patient care: Diagnosing and treating acute and chronic illnesses for a panel of up to 450 seniors. Health coaching: Acting as a health coach to help patients set and achieve health goals. Team leadership: Leading a care team including care promoters, facilitators, and coordinators. Collaboration: Working with hospitalists, specialists, and transitional care teams. Quality and leadership: Taking accountability for quality metrics and participating in clinic leadership and quality improvement initiatives. Qualifications Education: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty. Experience: Minimum of 2 years in a clinical setting. Licensure: Active state medical license and DEA license. Certification: Board certification is preferred.
