Compassionate Medical Social Worker - Home Health

Healthy Living at Home - Monterey, LLC is in search for a compassionate and experienced MEDICAL SOCIAL WORKER (MSW) to provide assessment, evaluation, and referral to services for our Home Health patients within the greater Monterey and Salinas County areas. Position Overview The medical social worker is responsible for the implementation of standards of care for medical social work services. Identifies and analyzes the social/emotional factors underlying client illness and communicates these factors to the health team. Assists patients and their families in understanding and accepting treatment of the temporary effects of illness. Assists with financial, community resources so that facilitation of recovery is acquired. Essential Job Functions Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse. Carries out social evaluations and plans intervention based on evaluation findings. Maintains clinical records on all patients referred to social work. Provides information and referral services for patients and families/caregivers regarding practical and environmental needs. Provides information to patients or families/caregivers and community agencies. Serves as liaison between patients or families/caregivers and community agencies. Maintains collaborative relationships with organization personnel to support patient care. Maintains and develops contracts with public and private agencies as resources for patient and organization personnel. Participates in the development of the total plan of care and case conferences as required. Assists physician and other team members in understanding significant social and emotional factors related to health problems. Participates in discharge planning. Basic Qualifications Master’s degree in social work accredited by the Council on Social Work Education. One (1) year experience as a Medical Social Worker (MSW) One (1) year experience working with patients in Home Health Demonstrates good verbal and written communication, and organization skills. Current CPR certification. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. Physical Requirements Typical functions of this role include standing, sitting and walking a majority of the time. There are occasions when kneeling, stooping, bending or reaching may be necessary. The employee must be able to lift and/or move up to 10 lbs on a regular basis. There may be occasions in which the employee must be able to lift or move up to 40 lbs. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
