Bilingual Vietnamese/English Health Home Care Coordinator

KWA Cares in Tacoma, WA is seeking a Health Home Care Coordinator who is fluent in Vietnamese and English. You will support members with chronic conditions by delivering patient-centered care coordination and reducing gaps across medical, behavioral health, and social services. This onsite, full-time role runs Monday to Friday, 8:30am–5pm, with a competitive hourly range of $21.00–$23.00. You will use a Health Action Plan and ongoing case management to ensure clients receive the right care at #J-18808-Ljbffr KWA Cares
