Job Summary

Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Assess patients and coordinate care to meet their discharge planning needs.
• Organize and participate in patient and family meetings to discuss care plans.
• Develop treatment plans in collaboration with interdisciplinary teams.
• Manage case loads effectively, maintaining patient ratios of 1:25 during weekdays and 1:45 during weekends.
• Coordinate with insurance providers (HMOs, IPAs) to ensure appropriate care authorizations.
• Implement discharge plans including DME, SNF, and ambulance transport arrangements.

Qualifications

• Current and valid RN license in the state of California, with pending license accepted upon start date.
• At least 2 years of Acute Care Case Management experience required.
• Experience with Epic charting system preferred.
• Ability to handle a case load of up to 25 patients in acute care settings.
• Strong organizational skills and ability to work independently and as part of a team.
• Commitment to meeting department scheduling needs, including every other weekend rotation.
