About the Facility

Adamsville Healthcare & Rehab Center is a 140-bed skilled nursing facility offering short-term rehab, long-term care, and outpatient therapy services.
The facility also includes a 46-bed secure memory care unit with a state-of-the-art sensory room.

Adamsville, Tennessee, located in Hardin County, is a thriving community with many amenities and all the charm of Southern living, without the big-city problems and a low cost of living. The town offers a slower pace, with a focus on community events, family, and outdoor activities. The Adamsville location offers easy access to Memphis, Nashville, and Jackson, Tennessee; Corinth and Tupelo, Mississippi; and Florence, Alabama, while the community has plenty to offer to keep folks close to home.

Responsibilities

• Review clinical information and evaluate its impact on the assessment and treatment process.
• Conduct comprehensive functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable functional goals.
• Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly.
• Revise treatment plans based on patient performance and progress.
• Supervise and guide therapy assistants, ensuring proper treatment techniques and the delivery of care.
• Educate residents, families, and caregivers about exercises and therapeutic procedures to support carryover.
• Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes.
• Assist with discharge planning and maintain knowledge of community resources.
• Participate in care conferences, meetings, and quality improvement initiatives.
• Maintain clinical competence through continuing education opportunities.
• Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines.
• Support the company's mission, vision, and professional expectations.
• Maintain expected productivity levels and demonstrate excellent teamwork and communication skills.

Qualifications

• Bachelor's or Master's Degree in Occupational Therapy
• Active OT license in the state of practice
• BLS/CPR certification (current or obtained during orientation)
• Strong clinical reasoning, communication, and documentation skills
• Ability to collaborate effectively within an interdisciplinary team
