Job Description

Job Opportunity: Nurse Navigator

Our organization is looking for a dedicated Nurse Navigator to assist clients within our outpatient behavioral health programs. This role is centered around patient care and involves a variety of responsibilities aimed at enhancing the treatment experience.

Key Responsibilities

Coordinate comprehensive care for clients.
Conduct nursing assessments to evaluate patient needs.
Guide individuals and their families through treatment options, available resources, and follow-up services.
Collaborate with a multidisciplinary team to connect patients with counseling, psychiatric services, and community support.
Encourage patient engagement in their care journey and monitor their progress throughout treatment.

Requirements and Qualifications

Registered Nurse (RN) license.
Experience in behavioral health or related fields is preferred.
Strong communication and interpersonal skills.
Ability to work effectively within a team-oriented environment.
