Collaborate with primary care providers to develop individualized care plans. Address and continuously reassess levels of care to ensure appropriateness and cost-efficiency. Coordinate care transitions with physicians, social workers, and discharge planners. Maintain detailed records of clinical, functional, and fiscal outcomes during management.
