Clinical Social WorkerThe Clinical Social Worker provides Social Services to patients to maximize the patients' well-being and improve outcomes. Also works as part of an Interdisciplinary Team to maximize patient care and support the company to meet those goals of the QAPI activities. PRIMARY DUTIES / ACTIVITIES:Conduct assessments and devise plans of care to help improve adjustment to dialysisAssist with home management in conjunction with the Inter-Disciplinary Team (IDT)Communicate effectively with the IDT about patient issues, intervention and goalsConsult with doctors, therapists and medical professionalsWork with admissions to ensure insurance issues are addressedActively participate in activities that elevate clinical outcomes throughout the QAPI processWork with Crown web for submission of dataParticipate in individual & group projects as directed by managementOther duties, as assignedCUSTOMER SERVICE:Responsible for supporting the values and customer service standards of HDS.Accountable for outstanding customer service to all staff, clients and vendorsDevelops and maintains effective relationships through effective and timely communication.Initiates action to respond, resolve and follow up regarding customer service issues with all customers in a timely mannerEXPERIENCE AND REQUIRED SKILLS:Must be PC proficient and able to thrive in a fast-paced setting; experience in Microsoft Office Suite: Outlook, Word and Excel a must;Good presentation & communication skillsMust be productive & resourceful with the ability to meet deadlinesOTHER QUALIFICATIONSMust be a self-starter who can work independently yet function in a highly collaborative, fast-paced team environment.Ability to multi-task, work under pressure and meet deadlines effectivelyExcellent written and oral communication skillsStrong data entry, organizational and interpersonal skillsHigh integrity, team player with patient service focus and flexibilityEDUCATION:Minimum 1 year of experience working as a renal social workerMasters of Social Work, requiredLicensed Social Worker (LSW)Voluntary Benefits:Medical, Dental & Vision InsuranceHospital Indemnity, Critical Illness, Accident Insurance & Long-Term DisabilityVoluntary Life and AD&DFlexible Spending Account (FSA) & Health Savings Account (HSA)Company-Paid Benefits:Short-Term Disability & Basic Life/AD&D (1× salary)Employee Assistance Program (EAP) & LifeMart discounts401(k) with Company MatchPaid Time Off (PTO) & Extended Sick Leave (ESL)PHYSICAL DEMANDS AND WORKING CONDITIONS: The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.
