About the Role:

We are seeking a Licensed Practical Nurse (LPN) to join our team at Vivo Healthcare Taylor Manor ALF in Jacksonville, FL. As an LPN, you will be responsible for providing basic nursing care to our residents, including:

• Observing and reporting changes in resident condition
• Administering medications and treatments as prescribed by physicians
• Maintaining accurate and detailed records of resident care
• Developing and implementing individualized care plans
• Assisting with daily living activities, such as bathing, dressing, and grooming

Responsibilities:

• Deliver high-quality care and support to residents, meeting their physical, emotional, and social needs
• Collaborate with other healthcare professionals to ensure comprehensive care
• Maintain a safe and clean environment, adhering to infection control practices
• Participate in quality improvement initiatives to enhance resident care and outcomes
• Communicate effectively with residents, families, and staff to ensure seamless care transitions

Requirements:

• Graduate of an accredited LPN program

• Current LPN license in the state of Florida

• Minimum 3 years of experience in long-term care or relevant healthcare setting

• Ability to lift and move patient loads, with assistance as needed

• Strong communication and teamwork skills
