Location: Logansport, IN (46947) Contract Length: 13 Weeks (Potential Extension) Schedule: Monday – Friday, 8:00 AM – 4:30 PM Weekly Hour Guarantee: 36 Hours Productivity Expectation: 30 Points per Week Employment Type: Travel or Local Contract Compensation: Local Pay: $56.00 – $60.00 per hour Travel Package: $2,600 – $2,850 per week Weekly Stipend: $1,246 Lodging: $770/week Meals & Incidentals (M&IE): $476/week Mileage reimbursement provided between patient visits. Position Overview: We are seeking an experienced 'Home Health Physical Therapist' to join a leading home health and hospice organization in Indiana. The Physical Therapist will provide skilled therapy services in patients' homes, conduct comprehensive evaluations, develop individualized treatment plans, and ensure high-quality patient care while maintaining compliance with Medicare and agency guidelines. Key Responsibilities:

• Complete comprehensive patient evaluations and establish individualized Physical Therapy plans of care.
• Perform Home Health OASIS visits, including: Start of Care (SOC) Resumption of Care (ROC) Evaluations Discharge Visits Deliver skilled physical therapy interventions in accordance with physician orders and agency policies. Assess patient progress and modify treatment plans as needed. Educate patients and caregivers regarding treatment plans, safety, mobility, and home exercise programs. Supervise Physical Therapist Assistants (PTAs) in accordance with company policies and state regulations. Maintain accurate and timely clinical documentation within 24 hours of each visit. Participate in branch meetings, case conferences, and interdisciplinary care coordination. Demonstrate flexibility in scheduling to meet patient care needs. Participate in weekend rotation and occasional holiday coverage as required.

Required Qualifications: Doctor of Physical Therapy (DPT) degree from an accredited program. Current, unrestricted Physical Therapy license in the State of Indiana. Recent Home Health experience required. Strong proficiency with HomeCare HomeBase (HCHB). Experience completing OASIS documentation. Thorough understanding of Medicare Home Health regulations and guidelines. Current CPR Certification. Valid Driver's License. Proof of current auto insurance. Reliable transportation.
