Job Title: Registered Nurse (RN) – Hospice Case Manager Location: Thornton, CO Employment Type: Full-Time Overview Our hospice team is seeking an experienced Registered Nurse (RN) to serve as a Hospice Case Manager supporting patients across home and facility settings. This role focuses on managing a caseload, coordinating interdisciplinary care, and delivering compassionate, patient-centered support throughout end‑of‑life care. Position Offers Monday–Friday schedule, flexible within standard office hours Competitive annual salary: $99,840-$108,160 (DOE) Autonomy in managing your patient caseload Collaborative interdisciplinary care team environment Meaningful work supporting patients and families Variety of care settings including homes and facilities Supportive leadership and care coordination structure Key Responsibilities Manage and coordinate a hospice patient caseload Perform comprehensive patient assessments and ongoing evaluations Develop, implement, and update individualized care plans Collaborate with physicians on symptom and pain management Coordinate care with interdisciplinary team members Provide skilled nursing care in home and facility settings Maintain accurate and timely patient documentation Qualifications Active Registered Nurse (RN) license in Colorado Minimum of 2 years of hospice experience Strong clinical assessment and care planning skills Excellent communication and collaboration abilities Reliable transportation for patient visits JOB-11402 Johnathan Gomez One Stop Recruiting is an equal opportunity employer.
