Roles & Responsibilities
- Evaluate patients' physical therapy needs and develop individualized treatment plans
- Implement therapeutic interventions to improve mobility, strength, and functional independence
- Document patient progress and adjust treatment plans as needed
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Educate patients and their families on treatment strategies and home exercise programs
- Participate in care planning meetings and contribute to patient goals
Qualifications
- Active Physical Therapist licensure in the state of Oklahoma
- Current BLS/CPR certification
- Experience in skilled nursing or long-term care settings preferred
- Strong clinical reasoning and documentation skills
- Compassionate and patient-centered approach
Knowledge & Skills
- Knowledge of physical therapy modalities and evidence-based practices
- Proficiency in patient assessment and treatment planning
- Excellent communication and interpersonal skills
- Ability to work effectively within a multidisciplinary team
- Familiarity with electronic medical records (EMR) systems
