Join our dedicated team at a leading community health center in NYC, where we are committed to providing exceptional care to our diverse patient population. We are seeking a compassionate and skilled healthcare professional to deliver high-quality primary care services, focusing on both preventive and comprehensive patient care.
Job Responsibilities- Conduct thorough health assessments to diagnose both acute and chronic conditions.
- Develop, coordinate, and manage individualized treatment plans tailored to each patient’s needs.
- Prescribe medications and therapies in accordance with state and federal guidelines to ensure safe and effective patient care.
- Collaborate with interdisciplinary teams to promote continuity of care and enhance patient education.
- Clinical leadership experience preferred.
- Valid NY State License.
- Board Certified in Family Medicine.
- Current Basic Cardiac Life Support (BCLS) certification.
- Current DEA Certificate.
