Visiting Nurse and Hospice for VT & NH

VNH is an established leader in home health care and hospice, serving this community since 1907. As a non-profit, our only goal is helping people and providing you with the right team and resources to care for your patients. Responsibilities Assess, plan, implement, coordinate, and evaluate the care for each patient and develop appropriate treatment interventions, evaluation of outcomes, and utilization of services to ensure that patient care needs are met. Supervise other home care staff as necessary. Qualifications Must possess an active RN license in both VT and NH. ASN required, BSN or MSN preferred. Nights, Weekend, Holiday and on-call rotation required. Education Associates or better in Nursing. BSN or higher preferred.
