Job Details Job Location: Oak - Phoenix, AZ 85008. Position Type: Full Time. Education Level: High School Diploma / GED. Salary Range: $17.87 - $19.98 hourly. Travel Percentage: Up to 50%. Job Shift: Day Shift (M-F 8:30a - 5:30p). Hours: 40 hours per week. Company: Terros Health. Duties and Responsibilities Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach those goals. Completes assessments, crisis plans, and individualized service plans. Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with members, their families, behavioral health, medical and dental providers, community resources, and others to ensure all services prescribed in the individualized service plan are implemented. Conducts outreach and follow‑up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place. Performs all case management functions associated with a caseload including completing the assessment and service planning processes and identifying the need for further or specialty evaluations. Pursues best practice outcomes for persons with mental illness including continuing education, employment, independent housing, and community tenure. Additional $1.50/hr SMI differential. Qualifications GED or high school diploma with a minimum of 1.5 years of behavioral health experience. An associate’s degree in the field of medical or behavioral health. A bachelor’s degree (not in the field of behavioral health) and 1 year of behavioral health experience. A bachelor’s degree in the field of behavioral health. A master’s degree or higher in the field of behavioral health. Computer skills: Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferred. Valid Arizona driver’s license and minimum 4 years driving experience. Valid Arizona Fingerprint Clearance card (Level 1) or application within 7 working days of employment. Interpersonal skills for professional and positive relationships with co‑workers, members, families, and community members. Good communication skills: non‑judgmental approach, effective listening, positive interactions. Basic understanding of the DSM‑V preferred. Certified Case Manager strongly preferred.
