Job Description

We are seeking a dedicated professional to join our team in a role focused on case management within an acute care setting. This position is critical in coordinating patient care and ensuring compliance with established guidelines and procedures.

Key Responsibilities

• Manage a caseload of up to 25 patients, providing effective care coordination and discharge planning.
• Ensure compliance with HIPAA guidelines and CMS regulations.
• Collaborate with interdisciplinary teams, including social services, to help patients navigate their care.
• Handle admissions, denials, and appeals related to patient care.
• Conduct needs assessments and manage prior authorizations as required.

Weekend Requirements

This role requires the candidate to work every other weekend as part of their schedule.

Qualifications

• Minimum of 2 years of acute care experience is required.
• Experience with HMOs and IPAs is essential.
• Experience with advance directives and the protective services process is preferred.
• Knowledge of the Joint Commission standards and core measures is beneficial.
• Strong skills in care coordination, patient communication, and case management.

Essential Skills

• Proficiency in electronic health record systems; experience with Epic is required.
• Ability to float between similar units based on departmental needs.
• Understanding of pediatric, adult, and geriatric care considerations.
