Home Care Intake Coordinator- St Louis
Job Summary
Rockaway Home Care, a division of Foundation For the Elderly, is seeking a compassionate and organized Home Care Intake Coordinator to join our team in St. Louis, Missouri. As a vital member of our home health services team, you will play a critical role in ensuring a seamless intake process for our clients and their families. Responsibilities
Coordinate the intake process for new clients, including patient assessments, insurance verifications, and paperwork completion
Collaborate with our care team to develop personalized care plans that meet the unique needs of each client
Communicate effectively with clients, families, and healthcare providers to ensure a smooth transition into our home care services
Maintain accurate and up-to-date records of client information, care plans, and service delivery
Identify and address any issues or concerns that may arise during the intake process
Demonstrate a commitment to our company slogan, "You're not a patient, you're family," by treating each client with dignity, respect, and compassion
Requirements
High school diploma or equivalent required; degree in a related field preferred
Minimum of 1 year of experience in a healthcare setting or home care organization
Strong communication and organizational skills, with the ability to work effectively in a fast-paced environment
Proficiency in electronic medical records and Microsoft Office
Familiarity with state and federal regulations governing home care services
Ability to work independently and as part of a team
