Community Outreach Case Worker — Mental Health Support

Aspire Health and Community Services is seeking an Outreach Case Worker to support adults in the community with chronic behavioral health issues. You will work with a multidisciplinary team to assess needs, coordinate services, and advocate for access to resources. This is a full-time role with schedule Tue–Sat and shift coverage.
