Marketing Referral Specialist

Position Summary The Referral Specialist is responsible for driving new client admissions by (1) In-person visits with referral sources (2) Creating and strengthening community partnerships/relationships (3) Providing education on services and feedback about client outcomes (4) Completing initial assessments for admission (5) Successfully managing communication between referral sources and home office. Responsibilities Summary Create Referral Opportunities Visit Hospitals/CCRCs/SNF/AL/IL/etc and establish effective rapport with physicians, social workers, discharge planners, therapists, and other referral sources Participate in external marketing, including building relationships with referral sources, hosting and supporting marketing functions sponsored by “power partners” to engage in community networking and inter-personal connections Integrate efforts with community marketing team to ensure efforts are coordinated and in line with the team's goals Seek new markets (LTC Insurance, Estate Attorneys, etc) in the community to promote education and depth of influence Educate and Inform Develop and deliver educational in-services to referral sources Attend weekly case conferences and provide feedback/reports on client's progress Acts as liaison to referral partners to report client's progress toward goals, response to assistance, or updated treatment plans Serves as a resource to hospital planners to ensure smooth transition from hospital to home Plan & Document In cooperation with the Administrator and Client Supervisor, assess the needs of the marketing program and develop a marketing plan of action with weekly/monthly/quarterly/yearly goals Create and present business plan to target hospitals, Assisted Living Facilities and other key referral sources Consultations/Assessments Be readily available to complete consultations/assessments. Consultations/Assessments may occur after regular business and on weekends Build initial rapport and become a trusted advisor to families Ensure new client paperwork is submitted to in-house team within 24 hours of admission Clearly communicate all roles involved in the outcomes of the client Maintain transparency with pricing and cost of services prior to start of services Communication is KEY! Follow all policies and procedures for communication of new starts to in-house team Outcomes What defines success in this role: Meeting quota for client referrals - 50% Conversion Rate > 70% - 20% Territory Growth (# of active referral resources) >20% quarterly - 10% Verified successful communication with in-office staff - 20% Qualifications Minimum 2 years of Home Health, Hospice or Durable Medical Equipment Sales experience and an understanding of healthcare operations is an essential requirement for this position Current, active book of business required Effective, responsive interpersonal skills in relating to nurses, social workers, case managers physicians, therapists, other health specialists in the community, management, and co-workers both over the phone and in person #J-18808-Ljbffr BrightCare HomeCare
