Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction; 60-minute evaluations with 30-minute follow-up appointments
• Delegate and direct all discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support return to community activities
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Adhere to discipline-specific State Practice Act and all applicable regulatory requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements
• Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned, providing appropriate supervision and completing student evaluations
• Actively pursue professional development through continuing education, self-study, and involvement in professional activities and organizations

Required Qualifications

• Active Physical Therapist license in good standing in the state of Colorado
• Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans
• Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
• Knowledge of and adherence to discipline-specific State Practice Act and professional code of conduct

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring physical therapy students
• Familiarity with performance improvement methodologies and quality initiatives
