RN - Case Manager

Registered Nurse Case ManagerThis position requires working in an acute care facility. Travelers are expected to float between nearby facilities if needed. Candidates must be able to work every other weekend to meet departmental needs. Flexibility regarding scheduling and time off requests is essential. Must commit to working three of the five major holidays. Detailed patient care responsibilities include care coordination and discharge planning. Experience with HMOs and IPAs is beneficial. Willingness to participate in the development of treatment plans is expected. Candidates must be comfortable working with interdisciplinary support teams. Basic computer documentation skills are mandatory. Must have references covering at least one year within the last three years. This role requires a commitment to patient-centered care and collaboration with healthcare professionals. Candidates should demonstrate effective communication skills and the ability to manage multiple priorities. Adhering to facility standards and regulations is critical for success in this role.Experience: Minimum 2 years in Acute Care Case Management/Discharge Planning License: California state license required; pending licenses accepted but must be licensed upon start date Certifications: Current nursing certifications relevant to the specialty Must-Have: Client experience is required; behavioral health experience does not apply. Candidates must have experience assessing patients, care coordination, discharge planning, and utilization review. Ability to organize and participate in family meetings is necessary. Experience with knowledge of adoption/foster care, drug/alcohol abuse treatment options, and patients experiencing homelessness is essential.
