Schedule: Full Time Positions Available: 2 Job Location Type: On-site Job Location: Blue Mountain Hospital, John Day, OR Position Summary As a Physical Therapist at Blue Mountain Hospital District (BMHD), you will join the rehabilitation department team that includes SLP, occupational and physical therapy services. Inpatient/Home-Based Position This position is primarily in the Home-Based setting. The Physical Therapist evaluates care plans and administers comprehensive physical therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative physical therapy as prescribed by a physician. Inpatient/Outpatient Position This position is primarily outpatient with the ability to work partial hours in inpatient care. In both settings you will work 1-on-1 with patients and will have time built into your schedule for documentation. Continuing education is encouraged with a yearly stipend and trainings built into therapists’ schedules. The ideal candidate will have broad experience/interest in neuro, vestibular, and ortho in the OP setting and will perform duties as assigned for improving a patient’s function, alleviating pain, and preventing disabilities through evidence-based evaluation, individualized plan of care, and skilled interventions. Essential Functions & Responsibilities Inpatient/Home-Based Position Performs accurate and comprehensive assessments Completes OASIS data sets for “therapy only” cases. Communicates changes in patients’ clinical status to other healthcare team members and HHA office staff verbally and in writing in a timely manner. Communicates physician orders to other healthcare team members in a timely, accurate, and thorough manner. Coordinates care planning and delivery with the physician, patient and family/caregivers, other healthcare team members and HHA staff to facilitate optimal patient outcomes. Assesses the patient for the need for durable medical equipment as well as other adaptive equipment needs. Develops appropriate home exercise programs tailored to the patient’s clinical status, needs and age. Provides supervision and mentorship to PT assistants. Submits verification of completed visits in accordance with HHA policy. Maintains current knowledge of community resources. Identifies and accesses appropriate external and internal resources to meet patient and/or family needs. Has ability to work independently and serves patients county-wide. Has time management skills, able to set up own appointments with patients. May have supervisory responsibilities for a Physical Therapy Assistant. Inpatient/Outpatient Position Performs thorough physical therapy evaluation, evidence-based plan of care, and skilled interventions, performing re-assessments and required paperwork in a timely manner. Administers physical therapy interventions specific to the patient’s need, may include manual therapy, neuromuscular re‑education, gait training, therapeutic activities, and therapeutic exercises to achieve functional goals set at evaluation. Ensures continuation of therapeutic plan following discharge by designing home exercise programs, instructing patients, families and caregivers in physical therapy recommendations, and recommending and/or providing assistive equipment. Coordinates care with physician referrals and directs care independently with self‑referrals. Participates effectively with the team of medical providers, understanding and knowing the role of inpatient, transitional care, and outpatient services. May be required to direct treatment given by a physical therapy assistant. Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, attending webinars, establishing professional networks, and participating in professional organizations. Performs any additional duties pertaining to the daily operations of the Rehabilitation Services Department as assigned by the Rehabilitation Services Manager. Maintains self‑motivation and sustains a high level of momentum. Prioritizes effectively when faced with requests, demands and deadlines. Has knowledge of customer service concepts. Is extremely detail oriented and performs within scope of practice. Communicates effectively in English, both verbally and in writing. Expresses excellent judgment, discretion and decision‑making abilities. Maintains a sensitive, fair and impartial approach to dealing with team members. Supports and maintains a culture of safety, quality and positive patient experience. Works with relevant regulatory requirements CMS, National Patient Safety Goals (NPSGs), and stays readiness related to healthcare or hospital standards. Is highly organized and can prioritize tasks. Education and Experience Requirements Graduate of an accredited school of physical therapy. For Medicare certified agencies: Graduate of a physical therapy curriculum approved by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council on Medical Education of the American Medical Association and the American Physical Therapy Association. Preferred Doctorate of Physical Medicine. Strong physical therapy skills for the neurologically impaired patient population. Experience in vestibular rehab. Experience in cardiac rehab. Experience treating chronic pain. Certification/Licensure (Required) Basic Life Support (BLS) certified by American Heart Association (AHA). PT License in good standing with the State of Oregon.
