Key Highlights Weekly pay: $2,181–$2,302 per week Guaranteed hours: 38 per week Contract duration: 13 weeks Shift: Days Start date: 09/14/2026 Setting: Long‑term care (LTC - Nursing Home) Responsibilities Perform initial PT evaluations for residents in the long‑term care setting and develop individualized treatment plans in collaboration with the interdisciplinary team. Deliver evidence‑based therapies addressing mobility, gait and balance, strength, range of motion, and activities of daily living to restore or maintain function. Monitor patient progress, document outcomes in the medical record, and adjust treatment plans as clinically indicated. Ensure patient and staff safety during therapy sessions, adhere to infection control standards, and follow facility policies. Educate patients and families on home exercise programs and activity modifications to support ongoing independence. Collaborate with physical therapy aides and other disciplines to optimize rehabilitation outcomes and coordinate care transitions.
