What Your Week Looks Like Travel within your assigned territory to deliver skilled nursing visits per each patient's physician‑ordered plan of care, under RN supervision Perform hands‑on treatments: wound care, medication administration, injections, IV therapy, and vital signs checks Monitor each patient's condition and report changes promptly to the RN or physician Educate patients and families on disease management, medications, safety, and self‑care so they can manage more independently between visits Supervise Home Health Aides on assigned care tasks Complete clinical documentation accurately and on time, in line with agency, state, and payer requirements Join interdisciplinary case conferences and quality improvement discussions Follow strict safety and infection control protocols on every visit What We're Looking For Graduate of an accredited practical or vocational nursing program Current, active LPN or LVN license in the state of Oregon One year of clinical experience, including one year specifically in home health Valid CPR certification, maintained throughout employment Valid driver's license and reliable transportation to travel to patient homes across the assigned territory Ready to Apply?
