PRN Physical Therapist- Outpatient Rehab

As a Physical Therapist at LCMC Health, you will champion multiple patients as they begin their rehabilitation journey, ensuring each receives individualized, compassionate care while collaborating closely with physicians, nurses, case managers, and families. Responsibilities Take the lead in prioritizing, coordinating, and delegating all physical rehabilitation care touchpoints for patients. Interact with other rehabilitation team members to coordinate goals and improve quality of care. Ensure physical therapy care and activities are consistent with the patient’s medical plan of care. Maintain and advocate a safe environment for all patients and staff. Monitor and engage with patients, families, and care team members for teaching opportunities, patient condition changes, and questions or concerns. Ensure continuity of care, efficient throughput, consistent communications, and an exceptional patient experience. Contribute to the team by assisting others as needed, without being asked. Provide guidance to new and experienced personnel and oversee ancillary personnel. Reassess and revise goals and treatment plans in accordance with patient status, program requirements, and CMS rules and regulations. Evaluate patient progress toward expected outcomes, documenting assessments, evaluations, and responses. Maintain appropriate data and contribute to departmental quality management activities, offering recommendations for systematic improvements based on data results. Demonstrate skill and accuracy in performing age‑appropriate assessments and document standardized assessment results. Act as a resource to Program Managers and Directors for accreditation standards set by CARF and other accrediting organizations. Qualifications B.S., Master’s, or Doctorate in Physical Therapy. Current license to practice physical therapy in the state of Louisiana. American Heart Association Basic Life Support/CPR certification (can be obtained on onboarding). Non‑Violent Crisis Intervention certification (can be obtained on onboarding). Preferred: one year of experience in a similar setting and additional certifications such as manual therapy, dry needling, vestibular rehabilitation, or women’s health.
