Outpatient Physical Therapist

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. Responsible for the delegation and direction of all discipline-specific therapy services for those assigned patients, the therapist ensures the design of functional treatment programs that allow for effective return to community activities. This role also involves serving as an educator for patients, families, students, coworkers, and self, while performing other professional duties associated with patient care and the development of discipline-specific therapy services.

Responsibilities

• Complete a comprehensive evaluation of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals focused on progressing patients toward discharge.
• Document medical records in a clear, concise, accurate, and timely manner per state and federal guidelines.
• Ensure care is provided under signed, valid physician orders and within insurance approval.
• Participate in student programs as assigned, providing a positive learning environment with proper supervision.
• Create an individualized professional development plan in collaboration with clinical management.
• Maintain professional licensure in good standing and adhere to the discipline-specific professional code of conduct.
• Demonstrate awareness of organizational performance improvement initiatives and participate accordingly.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for where work will be performed.

Schedule

This is a 5x8.0 shift position starting on October 2, 2026.
