Description
Do you have a bachelor’s degree and at least 1 year experience in geriatrics? Are you an LPN seeking work-life balance?
We're hiring experienced LPNs or Social Workers with a bachelor’s degree and at least one year of geriatric experience (think nursing home or home health).
No experience with ADvantage Case Management? That’s okay, we are willing to train the right person. We offer unparalleled flexibility in scheduling!
If you're a master at working independently and have top‑notch computer and typing skills, then this full‑time position with competitive pay could be your next dream job!
As an ADvantage Case Manager, you'll be responsible for initiating and monitoring service plans, setting achievable goals, and providing resource referrals to meet the needs of ADvantage Program Members.
Don't worry, though - your weekends and holidays are free! There is no on call required for this position.
