Housing Stability Case Manager

The Salvation Army Northern Division Anoka 360 Life Center in Blaine, MN is seeking a Case Manager to provide strength-based, person-centered services for housing stability. You will guide participants to access housing resources, develop goals, and coordinate supports with community partners.
Responsibilities include direct client assistance, completing assessments, maintaining documentation, and contributing to outreach while meeting program requirements.
