Experience: 2+ years in PT; 1 year in Home Care/Public Health. Skills: Proficiency in OASIS and BLS certification. Your Role: You’ll lead home care services for a dedicated caseload—performing assessments, initiating rehabilitative procedures, and coordinating with physicians. You’ll be the bridge between the hospital and a successful recovery at home. Pay Range: $80,080.00 USD - $107,777.00 USD The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time.
