Responsibilities

About the Role:

CUCS seeks a Case Manager for its Intensive Mobile Treatment (IMT) program, serving individuals with complex needs across homeless, criminal justice, and behavioral health systems. IMT provides flexible, continuous, mobile care to improve quality of life regardless of a client's location (shelter, street, housing, hospital, or jail).

Key Responsibilities:

• Provide person-centered case management, counseling, recovery planning, and crisis intervention.

• Complete housing applications and assist with placement, employment, and public benefits.

• Conduct community outreach and escort clients to appointments across NYC.

• Advocate for clients and liaise with criminal justice, shelter, and medical providers.

• Collaborate with an interdisciplinary team to ensure continuity of care and meet contract standards.

Requirements:

• Education: Bachelor’s degree in Social Work, Psychology, Mental Health Counseling, Criminal Justice, or related human services field required.

• Experience/Skills: Experience with substance use, mental illness, and trauma preferred; strong assessment, communication, and computer skills.

• Travel: Field-based position; must be able to travel across all 5 NYC boroughs. Spanish/English bilingual skills preferred.

Benefits:
