This is a 13-week contract position.

Job Description\n

Join our home health team providing exceptional care to patients in their homes. You will treat a diverse patient population with common diagnoses including general orthopedic conditions, generalized weakness, strengthening needs, and cardiac rehabilitation.

Responsibilities\n

\n
• Conduct evaluations and develop treatment plans for patients.
\n
• Implement treatment protocols in accordance with patient needs.
\n
• Document patient care and progress using the Epic documentation system.
\n
• Communicate effectively with patients, families, and healthcare team members.
\n

Required Experience / Certifications / Licensure\n

\n
• BLS Certification
\n
• Valid Physical Therapy license for the state of West Virginia
\n
• Experience in home health preferred but not required.
\n
