As a Care Coordinator, you'll play a vital role in ensuring our clients receive compassionate, reliable care while supporting caregivers and collaborating with our internal team. What You'll Do

Coordinate the start of care for new clients and ensure a smooth onboarding process. Maintain accurate client and caregiver records in our software systems. Communicate with clients, families, caregivers, and healthcare professionals regarding care needs and updates. Monitor client satisfaction through routine follow-up calls and quality assurance check-ins. Assist with caregiver-client matching based on client needs and caregiver qualifications. Track required documentation and ensure compliance with company policies and state regulations. Support caregivers with questions, concerns, and ongoing communication. Collaborate with office staff to ensure continuity of care and excellent customer service. Assist with care plan updates and documentation as needed. What We're Looking For

Previous experience in home care, healthcare coordination, case management, or a related field preferred. Excellent communication and interpersonal skills. Strong organizational skills with exceptional attention to detail. Ability to prioritize multiple tasks in a fast-paced office environment. Proficiency with Microsoft Office and the ability to learn new software systems. Professional, compassionate, and customer-service oriented. High school diploma or equivalent required; additional healthcare or administrative experience preferred. What We Offer
