Job Description
Title: LPN - Long Term Care

Schedule: Minimal RTO EOW

Requirements:
- Must possess an active LPN license.
- Minimum of 2 years of nursing experience, with at least 2+ travel assignments prior to starting.
- Familiar with Point Click Care.
- Experience in Long Term Care and Short Term Rehab.
- Prior experience with admissions.
- Familiarity with a nurse to patient ratio of approximately 1/17 for Long Term Care and 1/13 for Short Term Rehab during days and PM shifts; 1 nurse per unit ratio of about 1/28-30 during night shifts.
- Experience in facilities with a similar size (149 beds).

Roles & Responsibilities:
- Provide nursing care to patients in Long Term Care and Short Term Rehabilitation settings.
- Collaborate with healthcare team members to ensure effective patient care.
- Conduct patient assessments and develop individualized care plans.
- Assist with admissions and ensure proper documentation is maintained.
- Monitor and report changes in patient conditions to appropriate personnel.
- Maintain compliance with facility policies and procedures.

Compliance / Notes:
- Must adhere to orientation rules and be prepared for background checks and drug screens.
- Must follow dress code requirements as specified.
