RN - Case Manager

RN Care Manager

We currently have an opportunity for experienced RN Care Managers for a 13-week assignment. These are 40-hour per week positions with variable shifts of either 8:30 am to 4:30 pm or four ten-hour shifts from 7:30 am to 5:00 pm. There is no call, but weekend and holiday rotation is required. The RN Care Manager will coordinate concurrent utilization management activities house-wide. This position serves as the main link between the patient, healthcare team, organization, and payors during the patient's hospital stay. Responsibilities include conducting assessments of patient needs, identifying necessary resources for patient transitions, and securing those resources. Collaboration, education, and leadership within an interdisciplinary team during daily care rounds, care conferences, and care planning are essential. Excellent communication skills in written, verbal, and electronic systems are required. The ability to work with community organizations for securing durable medical equipment and arranging transportation is necessary. Participation in managing the length of stay for an inpatient hospital population is expected. Knowledge of hospital reimbursement methods and quality measures applicable to care management is important.
