RN Case Manager
HUCKEYE HEALTH SERVICES LLC
Shift: Night-shift, 4x10-hour shifts (11:00 PM - 7:30 AM), every other weekend required. Responsibilities Provide case management for patients in a Level 1 trauma center Emergency Department. Coordinate care for acute inpatient cases, ensuring efficient and quality outcomes. Utilize EPIC documentation system for accurate record-keeping. Collaborate with multidisciplinary teams for discharge planning and patient advocacy. Qualifications Minimum of 3 years of acute inpatient case management experience in a Level 1 Trauma Center. Flu vaccine required (medical or religious exemptions accepted). COVID-19 vaccine not required. Compensation Overtime: After 12 hours/day and 60 hours/week. #J-18808-Ljbffr
