Job Summary

Our client is seeking a Clinical Pharmacist to join their team. The primary responsibilities of this role include evaluating patient-specific data to determine eligibility for home care, developing and monitoring individualized pharmacy care plans, providing education and consultation to patients and healthcare providers, and managing medications and equipment to ensure patient safety and compliance with regulations.

Key Responsibilities

• Evaluate patient-specific data to determine eligibility for home care.
• Ensure prescribed therapies align with home care safety goals and medical necessity.
• Develop and monitor individualized pharmacy care plans.
• Proactively recommend modifications to therapy or monitoring protocols to optimize outcomes.
• Train patients and caregivers on medication administration using clear, effective communication.
• Provide evidence-based drug information and clinical support to healthcare providers.
• Accurately prepare and dispense medications in accordance with safety and quality assurance standards.
• Maintain and troubleshoot infusion/administration equipment, providing technical guidance as needed.
• Adhere to all federal and state laws, including DEA regulations and HIPAA confidentiality standards.
• Maintain strict compliance with accreditation, legal, and safety requirements.

Qualifications

• WI Pharmacist License
• At least 1 year of experience
• IV experience

Experience

• At least 1 year of experience in the relevant field

Additional Requirements

This position requires availability to work Monday to Friday from 8:30 AM to 5:00 PM, with on-call responsibilities every third weekend.
