Responsibilities
- Conduct OT assessments and develop individualized treatment plans for adult residents in the SNF/acute care context
- Implement therapeutic activities to improve or restore functional independence and safety with daily tasks
- Document evaluation findings, progress, and discharge planning notes; communicate with the interdisciplinary care team
- Educate patients and families on therapy goals, home exercise plans, safety strategies, and adaptive equipment
- Monitor patient responses to treatment and adjust plans to optimize outcomes
- Collaborate with nursing, rehabilitation, and therapy staff to coordinate care and ensure smooth transitions across settings

Requirements
- Active Occupational Therapy license in California (or licensure eligibility per state regulations)
- Degree from an ACOTE-accredited OT program
- Relevant experience in acute care or skilled nursing facility settings is preferred
- Ability to work 8:00–16:30, Monday through Friday, with guaranteed 35 hours per week
- Strong documentation and communication skills; ability to collaborate with an interdisciplinary team
- Willingness to commit to a 13-week travel assignment
