About the Role:
- Coordinating the delivery of home care services to clients, ensuring timely and efficient service provision
- Collaborating with clients, caregivers, and healthcare professionals to develop personalized care plans
- Managing client records, schedules, and communication
- Identifying and resolving any issues or concerns that may arise during service delivery
- Maintaining accurate records and reports, and adhering to confidentiality and HIPAA guidelines
- Providing exceptional customer service to clients and their families
- Participating in quality improvement initiatives to enhance the overall quality of care
