Registered Nurse (RN) Case Manager – Home HealthCompany: Home Health Care, Inc. Your clinical expertise becomes the foundation for safer recoveries and better patient outcomes.What You'll Be Doing Manage a home health patient caseload throughout the St. Paul service area Complete OASIS assessments and develop individualized care plans Perform skilled nursing visits and comprehensive clinical evaluations Coordinate care with physicians, therapists, social workers, and other healthcare professionals Provide medication management, disease education, and ongoing patient support Monitor patient progress and adjust care plans as needed Complete documentation using Homecare Homebase (HCHB) Help patients remain safely at home while reducing avoidable hospitalizationsWho We're Looking For Active Minnesota Registered Nurse (RN) license Three or more years of RN experience Home health experience preferred Excellent clinical assessment and critical thinking skills Strong communication and leadership abilities Comfortable managing an independent schedule Reliable transportation for travel throughout the St. Paul territoryWhy Home Health Care, Inc.
