Physical Therapist Assistant- Home Health

Assured Home Health, part of LHC Group, is seeking a Physical Therapist Assistant to perform treatment and provide exercise instruction and patient education, improving patients’ well‑being while following the established plan of care. Primary Responsibilities Follow state laws governing the provision of physical therapy in home care and perform only procedures qualified and trained for, and make recommendations for changes in the plan of care to the supervising Physical Therapist. Instruct and aid patients in active and passive exercise, muscle re‑education, gait, functional, ADL, transfer, safety, and prosthetic training. Observe, record, and report the patient’s response to treatment and changes in condition to the supervising PT, nurse supervisor, and/or physician, and coordinate care with other healthcare team members as appropriate. Instruct patients, families, caregivers, and other team members in therapy within the scope of the Physical Therapist Assistant. Function effectively in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client. Required Qualifications Current license, registration, or certification to assist in the practice of physical therapy in the state of Washington. Current driver’s license and vehicle insurance, or access to a dependable vehicle or public transportation. Availability to work a weekend rotation. Preferred Qualifications Current CPR certification or ability to obtain certification within 90 days of hire. Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility. Compensation & Benefits Pay range: Annual total cash target pay: $61,153 – $91,729 Per visit point: $35.28 – $52.92 Hourly rate: $29.40 – $44.10 The total cash compensation assumes full‑time employment (40 hours per week). It includes earnings from per visit point pay and hourly pay and may increase over time based on productivity and performance.
